The facility must retrain staff and correct storage practices to meet required standards.
Grass-fed ground beef packaged on Dec.
16 is under recall after tests detected E. coli O26 in products sold in Pennsylvania and five other states; USDA officials advise consumers to discard or return the items immediately to avoid health risks.
